If you are unable to print them, or you are having trouble completing any of them, we are happy to assist you in our office. Just tell the receptionist when you make your appointment so you can come in a few minutes earlier than your appointment time.

Insurance & Payment Information
Southeast Clinical Nutrition Centers, Inc. accepts Medicare, Blue Cross/Blue Shield, Humana, and Aetna insurances. Nutrition consultations for some diagnoses are covered by insurance with proper documentation from your physician. All co-payments and deductibles are expected to be paid at the time of service.
For patients without insurance or those with conditions not covered by a referred diagnosis, we also accept debit cards, Visa, MasterCard, and Discover in addition to personal checks and cash.
